Administrative Care Management
Administrative Care Management/Assessor (ACM) focuses on assessment and referral to treatment and Intensive Case Management Services.

ACM usually begins with an intake where basic information is gathered, such as:

  • Name, address, phone number and social security number
  • General health and treatment history
  • Employment status and income
  • Medical insurance coverage
  • Drug of choice and last use
During the intake, a date, time and place will be scheduled for an assessment.

A Care Manager/Assessor will work with you throughout your treatment assisting you with:

  • Level of care determination
  • Selection of an appropriate provider
  • Length of stay
  • Follow up or after care
During your assessment, your ACM will help identify areas of need for intensive case management services and offer to make an appointment for you to meet with the Intensive Case Manager. ICM services support treatment and healthy lifestyle changes and may be assessed at any time.
