Case Coordination

If at any time during an individual’s treatment episode, a “non-treatment” need is identified, they will be offered Case Coordination services.

Case Coordination is a service component of Case Management to assist those individuals who may lack the resources to meet the identified needs.

The underlying principle of Case Coordination is to increase an individual’s level of self-sufficiency in any of the following  areas:

        Housing  Child Care

        Mental Health        Physical Health

        Employment          Transportation

        Basic Needs         Life Skills

        Legal Services      Family/Social

        Educational / Vocational Training

A Case Coordinator will work with the individual to identify and set personal goals and to access services as he or she works towards achieving those goals, providing encouragement and advocacy along the way.
