Intensive Case Management
Intensive Case Management (ICM) is a client-directed service that addresses non-treatment community service needs.

The primary focus of an ICM are to evaluate your strengths and needs, to help you set goals, to link you to your needed services, to advocate for your rights, and to coach you as you work towards your identified goals.

ICM usually starts with an initial office visit. After immediate needs are considered, the first session will focus on:

  • A description of ICM and how it can help you
  • Completing and signing required forms
  • Identifying areas of focus through completion of an Inventory of Support Services (ISS)
  • Answering any questions or concerns you may have
  • Introducing the Service Plan which will be the road map for ICM services
Generally, ICM services will last as long as you participate and work towards your goals and have community service needs.

ICM requires a minimum number of contacts. The first month requires four (4) contacts; two (2) of which need to be in person. Additional months require two (2) contacts; one (1) of which needs to be in person. These are minimum contacts. If more are needed they can be scheduled.
